CANINE TRACKING NUMBER APPLICATION (CTN)

AND MEMBER APPLICATION

Due to the state of the economy, we have decided to waive membership fees for this year.  However, if you wish to participate in any championship series you must have a tracking number for that series for your dog.  You must renew your tracking number for each championship series in which you participate.  The tracking numbers are $10 per series.

By signing the Membership Application you understand that Performance Pups, Inc. is not a breed registry and that anyone can be denied membership at the sole discretion of the Directors.  You also agree to abide by the rules and regulations of Performance Pups, Inc. and further state that you shall not hold Performance Pups, Inc. or its officers, directors, employees, Judges, volunteers or trial sanctioning bodies liable for any injury which you may sustain at any Performance Pups, Inc. sponsored event.
